One drive member from a RAID-5 array was failed
 I pulled out the defective drive and pop in a brand new drive
 Strangely, it did not started to sync and after a few moment, the drive was identified as bad and I could observe a "Removed"; state from OMSA.
 I could also notice in the PERC firmware utility that the new drive contain a foreign RAID-0 configuration.

这是我为解决此问题而采取的步骤：

1.不要删除标记为坏/已删除的新驱动器

2.重新启动服务器，然后输入PERC固件实用程序(CTRL + R)

3.卸下标记为已损坏/已卸下的新驱动器(在控制器中)(不要担心，卸下新驱动器后，所有虚拟磁盘信息将在" Controller 0"下消失！)

4.按CTRL + ALT + DEL退出PERC固件

5.引导服务器，使新驱动器脱离托架

6.在阵列降级模式下成功启动服务器后，将新驱动器弹出

您的阵列重建应该很快开始！
